By signing below, I hereby give Meadowbrook Country Day Camp permission to have a third party conduct a criminal background check on me and to have its Insurance Company obtain an abstract from the NJ MVC (or any other appropriate agency if applicant’s driver's license was issued from any state other than NJ) if I am applying for a position which requires driving and for the Insurance Company to share its findings with MCDC. I also authorize MCDC to contact any previous employers and personal references.
